Overview

Mrs Davidson, a 70 year old patient with advanced COPD was admitted from home 10 days ago by emergency ambulance. On admission, the patient had evidence of a moderately severe infective exacerbation requiring IV antibiotics, oxygen, nebulisers, and steroids. The consultant decided the patient should be for ward level care. A junior doctor tried to have a discussion about DNACPR but Mrs Davidson refused to agree to one.

The Health Professional visits the patient at home after discharge to discuss an anticipatory care plan
